Before your new Catalog Request form will become active, you must enable it in the Advanced Settings section of Checkout & Registration Manager.

Enable Catalog Request

1. Sign in to your My Services page.
2. Click the Store link.
   
3. Go to Checkout & Registration Manager.
   
4. Go to Advanced Settings.
   
5. In the Catalog Request section, select the radio button beside Enable Catalog Request.
   
6. Click the Save button.
   
7. Publish your order settings.
